Dear Valued Patients,

Due to health reasons, I have permanently closed my Duncanville dental practice, effective October 5, 2026. I will not be able to attend to you professionally after that date.

Your continued dental care is important to me. I am pleased to share that I have partnered with Wheatland Dental Care to provide seamless, ongoing care for my patients. If you do not wish to transfer to Wheatland Dental Care, I encourage you to search your insurance plan for a list of covered dentists in your area.

Because your dental records are confidential, federal and Texas state laws require your formal written authorization before they can be released. To obtain a copy of your radiographs and charts, or to authorize a transfer to a provider other than Wheatland Dental Care, please instruct your new dental office to fax a signed records release request to (682) 254-9071.

I am sorry and saddened that I cannot continue as your dentist. It has been a privilege to care for you and your families. I extend to you my warmest wishes for your future health and happiness.
